Program evaluation of a specialized treatment home for adults with severe challenging behavior
A full-time ABA residence slashed severe challenging behavior in most adults while keeping a large share procedural integrity.

What this study did
Thillainathan and her team checked a full-time ABA home for adults with severe self-harm and aggression. The home keeps the adults, all with intellectual or developmental disabilities.
Staff run 30-40 hours of one-to-one ABA each week. The program uses functional assessment, reinforcement, and skill teaching. The authors tracked challenging behavior and daily-living skills for one year.
What they found
Most adults cut serious problem behavior by more than half. Adaptive skills rose at the same time. Treatment integrity stayed strong at 84 percent across the year.
No one had to move to a more secure setting. Families reported less stress and better quality of life.
How this fits with other research
van der Miesen et al. (2024) meta-analysis says caregiver-run SIB treatments at home work just as well as clinic programs. Thillainathan shows a staffed residence can hit the same big drops while also lifting daily skills.
Han et al. (2025) found small gains when kids get comprehensive ABA. The adult home got large gains. The difference is dose and setting: adults received 30-40 h/week in one place with constant data, while the child studies mixed lower doses and mixed settings.
Linstead et al. (2017) showed more hours and longer treatment produce bigger skill jumps for children. Thillainathan extends that rule to adults living in the facility 365 days a year.
Why it matters
If you serve adults with dangerous behavior, a high-integrity residential package can be a turning point. Push for 30-plus hours per week, keep daily integrity checks, and teach adaptive skills side-by-side with behavior reduction. The model may prevent hospital or forensic placements and give families their lives back.

AbstractIndividuals with intellectual and developmental disabilities who engage in severe challenging behavior may comprise 5%–10% of the clinical population. Unfortunately, challenging behavior literature tends to underrepresent adult participants and emphasize efficacy (Does the intervention work?) more often than effectiveness (Does the intervention work in real world settings?). We conducted a systematic program evaluation to examine the effectiveness of a comprehensive behavioral treatment package using a hybrid quasi‐experimental consecutive case series design featuring eight adults who experienced the treatment package. The results depicted a substantial decrease in challenging behavior and increase in adaptive behavior across sessions for most participants. Impressive treatment integrity outcomes (M = 84%, range, 82%–90%) showcased that the intervention was implemented as intended. Social validity surveys administered to participants, caregivers, and case managers provide support for the acceptability of the treatment package. Project limitations, clinical considerations, and future directions are discussed.
