What this study did

The team worked with children who have moderate to severe intellectual disability.

They compared two ways to keep the kids walking on a treadmill. One way used a peer model walking beside the child plus praise. The other way had only an adult standing behind the belt.

Each child tried both ways in an alternating-treatments design across short sessions.

What they found

When a peer model walked and the child earned praise, the kids tried to stop the treadmill far less often.

The simple change of adding a peer and praise made walking compliance much better than just having an adult nearby.

How this fits with other research

Cruz-Montecinos et al. (2024) looks like bad news: walking got worse when people with ID had to think while they walked. The studies do not clash. E et al. asked for simple steady walking. C et al. added a brain task on top of walking. Extra brain load, not the walking itself, hurt the gait.

Zhang et al. (2021) carried the idea further. They gave boys with severe ID a full year of semi-structured physical play and saw real skill gains. E et al. shows you can first get the child to stay on the belt; Lei shows long practice then builds real motor skill.

de Leeuw et al. (2024) pooled thirty digital-movement studies and found tech tools help. Their review now includes this 2008 treadmill-plus-model trick as an early low-tech example that still fits the "keep it fun and reinforced" theme.

03Original abstract

BACKGROUND: Individuals with Intellectual Disability (ID) exhibit reduced levels of compliance to exercise, including treadmill walking. The purpose of this study was to measure the effects of several training conditions on compliance to participation in treadmill walking of children with moderate to severe ID. METHOD: Criteria for compliance were the averaged number of times participants attempted to discontinue walking during two 5-min exercise sessions of treadmill walking at an intensity of 65-75% of predicted maximal HR. Fifteen children aged 5-11 with moderate to severe ID participated in the study. Training conditions were (a) close supervisor's position, (b) distant supervisor's position, (c) positive reinforcement, and (d) paired modeling. RESULTS: General linear mixed model statistics revealed significant differences in favor of the paired modeling and positive reinforcement compared to the other conditions. Leaning forward was the most frequent type of participants' attempt to stop exercising. CONCLUSIONS: Paired modeling and positive reinforcement should be considered within treadmill training programs for children with moderate to severe ID.
