What this study did

Three toddlers with autism got parent training in two steps. First, parents watched short videos about why problem behavior happens. Next, an early-intervention coach came to their homes and showed them exactly what to do.

The coach used the EISP model. That means she watched the child, found the reason for the behavior, and then taught the parent a simple plan. The study tracked how well parents followed the plan and how often challenging behavior happened.

What they found

Parents nailed the steps only after the coach arrived. Challenging behavior dropped fast and stayed low. The gains were big and lasted the whole study.

How this fits with other research

Clark et al. (2020) showed parents a feeding plan with just a handout and a three-minute video. Two of three kids still improved, but the gains were small. Yusuf et al. now show that adding live coaching turns small gains into large ones.

Austin et al. (2015) used home video feedback alone and saw softer benefits like less intrusive parenting. Yusuf’s team kept the home visits but layered on function-based coaching and got faster, clearer behavior drops.

Ouyang et al. (2024) pooled 32 trials and found that parent programs work best when you stack them. Their network meta-analysis supports Yusuf’s two-step recipe: start with video, then add real-time coaching.

Why it matters

If you run parent training, do not stop at videos or handouts. Schedule at least one in-home coaching visit where you watch the behavior, build a quick plan, and guide the parent through it. You will see sharper fidelity drops and faster behavior change in very young children with autism.

03Original abstract

PURPOSE: Challenging behaviors (CB), including aggression, tantrums, and noncompliance, are common among children with autism spectrum disorder (ASD) and can significantly affect family well-being and child development. This pilot study examined the effectiveness of a hybrid intervention model that combined early intervention service provider (EISP) coaching with parent-implemented, function-based behavior support strategies. METHOD: Three EISPs delivered structured home visits that included video-based behavioral training modules to three families of young autistic children exhibiting persistent CB. A nonconcurrent multiple-probe across parent-child dyads design was used to evaluate parent fidelity in implementing behavior support plans and corresponding changes in child CB. RESULTS: Results indicated that exposure to video modules alone produced minimal improvements in parent fidelity. However, the addition of individualized EISP coaching led to immediate and sustained increases in parent fidelity, accompanied by notable reductions in child CB across all participants. Social validity data reflected high parental satisfaction with the intervention procedures and outcomes. CONCLUSION: Findings support the feasibility and promise of scalable, family-centered models that integrate remote, video-based training with individualized in-home coaching. Such hybrid interventions may help bridge service gaps and enhance timely access to positive behavior support for families of young children with ASD.
