What this study did

LeSage et al. (1996) tracked the same kids for four years. All had low IQ plus ADHD. No therapy was given. The team wanted to know who still had big behavior problems later.

They watched for early signs of irritability. The goal was to see if those early signs predicted later trouble.

What they found

Most kids still showed serious acting-out and ADHD symptoms four years later. Early irritability was the clearest red flag. The problems did not fade on their own.

How this fits with other research

Eldevik et al. (2010) and Eldevik et al. (2006) seem to tell the opposite story. They gave low-IQ preschoolers behavioral intervention. IQ and daily skills rose sharply.

The key gap is the extra ADHD label. G’s kids had both low IQ and ADHD. Sigmund’s samples had low IQ with autism or no extra label. ABA helped the low-IQ-only groups. It was never tried in G’s ADHD mix.

Cornish et al. (2012) adds a parallel clue. In fragile-X boys, early attention scores predicted later skills. G found the same pattern with irritability instead of attention. Early markers matter in every developmental group.

Why it matters

If you serve kids with low IQ plus ADHD, do not wait for maturation to fix things. Track irritability at intake. Use brief rating scales each visit. When scores stay high, move fast: add parent training, token systems, or refer for medication review. The study says problems will stick around unless you act.

03Original abstract

Twenty-six of 30 participants (87%) who took part in a medication study for treatment of ADHD were followed up 2.9 to 4.8 years (Mean = 3.9 years) later. Parent ratings on the Aberrant Behavior Checklist Community (ABC-C) indicated continued problems on the acting-out subscales, and parent assessments on the Stony Brook Checklist-3R showed a high rate of difficulty on domains called ADHD. Conduct Disorder, and Separation Anxiety Disorder. A high percentage of children (69%) were taking psychotropic drugs, substantial numbers of their families had sought nonmedical treatments, children's friendships were often rudimentary, and a significant minority of children had disciplinary problems in school or difficulty with the law. Using Pearson correlations, we identified a number of initial variables that predicted follow-up parent ratings on the ABC-C and Stony Brook. The ABC-C Irritability subscale was useful in predicting both internalizing and externalizing problems at follow-up, whereas parent and teacher hyperactivity subscales failed to predict later hyperactivity. Children identified with both low intelligence and ADHD appear to have significant behavioral and emotional problems in their early adolescence, and there may be some important qualitative differences in the outcome of these youngsters as compared with that of children identified with ADHD and normal IQ.
